Hi Lynn The 40cm is the widest part of the chest, that is the width it will be after you've finish the increasing during the waist shaping. The bottom measurement on the diagram is the width after the decreasing for the waist and the top measurement is for after the increasing.

Decreasing tip, applies to armhole: All dec done from RS. Dec inside 4 garter sts. Dec as follows after 4 sts: slip 1 st as if to K, K1, psso Dec as follows before 4 sts: K2 tog. Increasing tip, applies to shoulder: Inc inside 4 garter sts. Inc 1 st by making 1 yo. On return row work yo into back of st to avoid a hole. (incorporate inc sts in moss st pattern as you go along). Back piece: Cast on 92-98-104-112-120-128 sts (includes 1 edge st each side) on needle size 5 mm [US 8]. Work 4 rows garter st, 2 rows stockinette st and continue in moss st – see above - until piece measures 5 cm [2"]. Now continue in stockinette st. Remember the knitting gauge! When piece measures 8 cm [3⅛"] dec 1 st each side on every 3 cm [1⅛"] a total of 14 times = 64-70-76-84-92-100 sts. When piece measures 50 cm [19¾"] work 8 rows garter st, 2 rows stockinette st and then complete piece in moss st. At the same time when piece measures 50 cm [19¾"] inc 1 st each side on every 5-5-4-4-3-3 cm [2"-2"-1½"-1½"-1⅛"-1⅛"] a total of 3-3-4-4-5-6 times = 70-76-84-92-102-112 sts. When piece measures 64-65-66-67-68-69 cm [25¼"-25½"-26"-26⅜"-26¾"-27⅛"] work 4 rows garter st on 7-7-9-9-11-11 sts each side – work remaining sts as before. On next row bind off 3-3-5-5-7-7 sts each side for armhole. Continue in moss st with 4 garter sts each side, at the same time dec – see Decreasing tip - 1 st on every other row a total of 3-5-6-9-12-16 times = 58-60-62-64-64-66 sts. When piece measures 73-76-77-81-83-86 cm [28¾"-30"-30⅜"-32"-32¾"-33⅞"] inc 1 st each side – see Increasing tip -, repeat the inc when piece measures 78-80-82-84-86-88 cm [30¾"- 31½"-32¼"-33"-33⅞"-34⅝"]. At the same time when piece measures 78-80-82-84-86-88 cm [30¾"-31½"-32¼"-33"-33⅞"-34⅝"] bind off the middle 14-16-18-18-18-20 sts for neck and dec to shape the neckline on every other row: 4 sts 2 times and 3 sts 1 time = 13-13-13-14-14-14 sts left on each shoulder.
