I think, by looking at the photo of the scarf, that I see pattern 4 twice. Pattern 3 is almost hidden because it comes directly after pattern 2. I think what you see before and after the "big holes" is pattern 4 twice. Try crocheting as described, and see if it comes out like the photo.
