Do the instructions for casting on 128 stitches (largest size) on pattern 6-16 infer that these are then to be joined in the round? Or knitted back and forth ? Hopefully it's the latter otherwise even the large size option is very small. Hopefully you can confirm.

I am making the Rainbow Unicorn for my granddaughter. I do not understand the directions for making the ears: "Knit 35, cast on 8 stitches for ear, knit 2, turn piece." The only time that I have ever cast on stitches in the middle of work is to make a buttonhole. Is the ear supposed to have a hole? Please advise, Mary
