Hi There is a section/tips in the beg of the pattern that explains how to crochet the dc's tog. The chart is showing when and how many. The number of chains you start with does include the 3 turning chains so nothing should be added. When working the last two rows you start by the shoulder side, crochet towards neck, turn and dec 1 dc in the beg of the row back. How this is done is also explained at the top of the pattern.
